A 34-year-old man is evaluated for asymptomatic skin changes in his groin that have been present for weeks to months. He has a long-standing history of plaque psoriasis without psoriatic arthritis, for which he uses a combination of topical medications. A few months ago, he developed redness and pruritus in his groin and tried using some of his psoriasis medications to treat the itching. For the plaque psoriasis, he uses clobetasol ointment twice daily on weekends and calcipotriene cream twice daily on weekdays.
On physical examination, vital signs are normal. There are large red plaques with silvery scale over the elbows, knees, and around the umbilicus. Skin findings are shown.
