A 45-year-old man is evaluated for a 1-year history of cough. The cough is described as episodic throughout the day and nonproductive. He believes his cough may worsen after a heavy meal. He reports no postnasal drip, shortness of breath, wheezing, chest pain, paroxysmal nocturnal dyspnea, or lower extremity edema, although he experiences occasional heartburn.
