She reports shortness of breath, generalized pain, fatigue, arthralgia, nausea, headaches, and migratory dysesthesia. She has been evaluated by multiple primary care and specialty physicians and has had more than 20 emergency department visits during this time; all studies have been normal. Trials of two different antidepressant medications were unsuccessful. Her quality of life has significantly declined over the past several years. Medical history is otherwise unremarkable.
