An 85-year-old man is being treated in the ICU for bacteremia secondary to a urinary tract infection. He was evaluated 2 days ago for intermittent confusion, weakness, bladder pain, and hematuria. Because of hemodynamic instability, he was admitted to the ICU. Intravenous fluids were given, and empiric meropenem was started, with resolution of his hypotension and fever. Medical history is otherwise noncontributory. He takes no other medications.
On physical examination, temperature is 37.7 °C (99.9 °F), blood pressure is 118/84 mm Hg, pulse rate is 96/min, and respiration rate is 18/min. Cardiac examination is normal. He has no flank or abdominal tenderness.
