A 36-year-old woman is evaluated in the ICU for a possible urinary tract infection. She was admitted for management of injuries sustained in a motor vehicle accident and has undergone multiple orthopedic surgical procedures. Yesterday, her temperature was elevated to 38.1 °C (100.6 °F) and a urinalysis with culture was ordered. She has no urinary symptoms. Medical history is otherwise unremarkable. Her only medication is vancomycin.
On physical examination, temperature is 36.8 °C (98.2 °F), blood pressure is 110/70 mm Hg, pulse rate is 88/min, and respiration rate is 16/min. BMI is 22. The general medical examination is unremarkable except for multiple contusions and surgical incision sites on the left upper extremity and bilateral lower extremities that are clean and dry.
