A 64-year-old man is evaluated for left-sided pleuritic chest pain. The discomfort is localized to the upper left chest and supraclavicular region. He is otherwise asymptomatic. Medical history is unremarkable except for 35-pack-year cigarette use; he continues to smoke. He takes no medications.
On physical examination, blood pressure is 132/72 mm Hg, pulse rate is 68/min, and respiration rate is 12/min. Multiple firm, nonmobile lymph nodes are present in the left supraclavicular space. The lungs are clear to auscultation bilaterally. The remainder of the examination is normal.
